The employee completed a written incident report and documented the following information regarding the incident. I was pulling truck with patient trays with left arm hit back of heal and jolted me forward. resulting in a pop. of my left shoulder. Contributing Factors as follows I was pulling instead of pushing and rushing under a lot of pressures with family member.
