While walking into the bathroom slipped and fell. She was able to stand up on her own and walked to supervisor office to report injury. No wet floor sign was present in bathroom.

Sprain of lumbar spine contusion of right hip.

Wet floor. Training education was done with the Housekeeping staff on the proper placement usage of wet floor signs and on slips trips falls.
