Employee was rigging material to relocate. Employee had picked up the load and moved it and was trying to separate the material to stage in another location.

Employee had placed the rigging device on the material but did not have a proper rigging on it. The material slipped from the rigging device the material fell to the floor. The rigging device flew back towards them and struck them in the face.
