Repairing Rollers

What happened

EE was instructed to replace bad rollers on the spot pack 3 conveyers. All motors and conveyers were locked out and EE was wearing a 3 point safety harness. When walking up to the top of the conveyer he bumped his head on the support beam and took a couple steps backwards and lost his balance causing him to fall off the side of the elevated Conveyers. EE had scratches on his right arm from trying to grab on to the side of the conveyer to catch himself. Fell from upper level.
