What happened

While hooking up a truck in the receiving area in [REDACTED] employee was tightening a nut that connects the receiving hose to the back of the milk tanker. He was tightening the hose with a wrench provided for the task. While tightening the nut he was pulling the wrench upward with his body weight and the wrench slipped off the nut causing him to lose balance and fall backwards on to the ground. When he fell he tried to catch himself and extended his left arm back and landed on it.
