What happened

During discharge from the main deck on the [REDACTED] Antwerp [REDACTED] was breaking out units on the main deck. She was spotting operator [REDACTED] through stanchions for him to leave the ship. His unit was facing the forward end of the ship and he had to do a U-turn and back up to get to the stern ramp. [REDACTED] had allegedly driven over a large bundle of lashing materials when turning around so another operator working the same deck [REDACTED] opted to U-turn further down the vessel toward the forward end of the ship to avoid getting stuck on the lashing material. As [REDACTED] was exiting the vessel she saw [REDACTED] near the end of the boom arm and [REDACTED] was falling to the deck. Witnesses confirm [REDACTED] was struck by the boom causing her to fall to the deck floor. The unit then passed over [REDACTED] with her placed between the tires.
