Before the incident

Operator was on the chemical catwalk to take samples for quality. Operator was not trained to complete this task as he was not leadership

Operator went to dip the tool into the tank to grab a sample. At the same time the hoist cycled and moved upwards out of the tank. The edge of the hanger caught his left forearm causing the laceration.
