Pouring concrete in column incasements on slab on grade

What happened

Worker was pouring a 4-6ft column encasement at grid line 6-R slab on grade as the worker was using the vibrator to vibrate concrete in the column in casement a steel wedge (5 lbs) left in the above steel column broke free and fell directly on top of the worker striking his hard hat. The worker fell backwards to the ground and was unconscious.
