[REDACTED] was closing a column form. But he had to open it again because it had damaged chamfer.

What happened

[REDACTED] was tied off at the top of the formed column to the rebar. Somehow the rebar at the bottom of the column began to bend and started to slowly bend to the side with [REDACTED] tied off to the top. He was unable to detach and came down with the column as it bent over and crushed his chest area.
