Per employee via incident report: I slipped and fell between the cars on ice. Had to be helped up by anouther employee and went to the ER. This happened at [REDACTED]. Pavement was wet. Did not see the black ice. A nurse from Ambulatory Surgery helped me up but I don 't remember name. [REDACTED] from breast care took me to the ER
