What happened

At approximately [REDACTED] QA supervisor [REDACTED] called for myself and first responder to processing 21. When i had arrived there with the first responder my baker [REDACTED] and [REDACTED] supervisor were standing there. [REDACTED] looked to be in alot of discomfort and holding the left side of his ribs. I had asked him what had happened and he told me that he slipped on some water that was by the mixer in proc 21. [REDACTED] supervisor first responder and [REDACTED] then headed towards the supervisors office while I stayed back to investigate the area in which the incident occurred. I found that there was a wet spot on the floor by the mixer and there were no caution wet signs out. I then proceeded to get the wet spot dried up and put caution floor wet signs up just to be safe. I then met the the others involved in the supervisors office to make a call to Ms. [REDACTED] the nurse to inform her of the incident.
