What happened

I opened an upper cabinet to replace cups in the water cooler. I leaned over to put them in the cup dispenser and when I stood back up I hit my head on the cabinet door. It hurt I saw stars closed my eyes took a few deep breaths and rubbed my head where it had hit. When I opened my eyes there was a slight shift in my depth perception. I got the pt water they had requested and completed the session. At the time it occurred I knew I needed to fill out this form the day of the event and I intended to do so but then kept forgetting to do so yesterday ([REDACTED]) and forgot to tell my manager. I am completing it today and informed my manager of the event as I started to do so.
