What happened

A Makeup operator was performing cleaning task on the belt conveyor under the rounder belt (Divider #2) specifically removing excess oil buildup. During the change over cleaning process the operator’s rag became entangled in the moving belt causing his hand to be pulled between the belt and the drive shaft.
