Before the incident

Employee thought rolls weighed a little more than they should have so he decided to tare out the scale again on Ext 1. After weighing the shaft and core EE returned shaft to the winder and began lowering the hoist. EE was watching the straps waiting for them to come down.

He never saw the straps move but then the t bar came into contact with his thumb resulting in a serious pinch.
