Before the incident

[REDACTED] was at the accumulaor belt by Line 8 and used a cylinder to activate the conveyors to move cans thru the German Frei equipment. [REDACTED] then went around the oven and over to the conveyor section after the beader to stop the cans.

What happened

[REDACTED] slipped on the concrete floor.

[REDACTED] struck the back left sife of her head on the conveyor frame as weall as the concrete floor. She also struck her left shoulder on the conveyer frame.
