driving a forklift

employee was driving a dump hopper with forklift to bring wood debris to a dump pile. The employee raised the forks with the dump hopper on it and exiting the forklift. The employee used the lever on the hopper to unlatch the hopper from the base to rotate the dump hopper and deposit the wood debris on the dump pile. When he did this the hopper slid forward on the forks and came off the forks. The side of the hopper struck the employee in the leg and caused a laceration.
