Making rounds on [REDACTED]. While reaching for the hardhat the operator struck their head struck on a hand wheel and valve stem for a 3 4-inch gate valve

What happened

While recording observations the operator dropped their pen. When they bent over to retrieve it their hard hat fell off and rolled under crude oil piping. The operator picked up the pen then proceeded under the crude oil piping to retrieve their hardhat. While reaching for the hardhat the operator struck their head struck on a hand wheel and valve stem for a 3 4-inch gate valve resulting in a laceration to their forehead.
