What happened

Employee began unloading material from the flatbed trailer using the ceiling hoist system. As the materials were being moved the hoist 's momentum caused material to swing. The truck driver did not remove accessory components at the rear of trailer which created a potential hazard. The swinging materials struck the accessory components resulting in the employee 's hand to be smashed.
