Before the incident

Employee was assigned to the de-lumping area where he was repsonsible for taking boxes off of a pallet opening them and pouring the contents into a de-lumping machine for futher processing. Employee was in the process of taking boxes off of the pallet when the injury occurred.

What happened

The pallet was 3x3x3 boxes high. The pallet was placed on a rotating table. The employee removed all boxes from the first two layers except for one corner of the pallet. The employee was working on the bottom layer and rotated the table and the single stack of 3 that he had left caused the pallet to become unstable and the boxes and pallet began to fall to the side. The employee attempted to catch the pallet and boxes.
