Mr. [REDACTED] stated that as he was walking on the south side 5th floor by room [REDACTED] he tripped and fell injuring his right knee and left foot. Mr. [REDACTED] tripped with a plastic towel strap that was on the floor. Mr. [REDACTED] refused any medical assistance and will continue his shift for the day.
