Employee alleged: On [REDACTED] I was assign to do 1:1 with the patient in [REDACTED] at about [REDACTED] an [REDACTED] a rapid response was called as the main door was opened by the doctors who was responding to the call the patient rush thru the opened door and went straight to the elevator as I tried to prevent him from taking elevator he started punch me to the extend that I fell chest down. Head left side of my face both knee. Finding; Tenderness to palate on left maxillary --- abrasion to right knee CT imaging of head face -----.
