Before the incident
Received pt confused and restrained bilateral wrist. Even with restraints pt was sliding down in the bed came to the pt room to do the pm care and assist the pca with vital signs as pt was restless and moving and kicking his leg. BP cuff was placed to his leg to do vitals Nurse was holding the leg with BP cuff to the leg in place. Meanwhile Pt kick the nurse. Nurse got the kick from pt to the left hand and the nurse ([REDACTED]) hand got twisted while trying to prevent the kick.

Even with restraints pt was sliding down in the bed came to the pt room to do the pm care and assist the pca with vital signs as pt was restless and moving and kicking his leg.
