Employee States: I went to wash my hands to help a couple customers. When I walked over to the hand was sink on my second step I slipped. I did not have anything to hold onto so I fell. I hit my head on the table as I fell and I also hurt my knee hip and elbow on the way down. email from Front End Manager [REDACTED] Good afternoon! [REDACTED] caught up with me today regarding her incident report. She said she was confused and forgot exactly what had happened when she fell in her department the other day. She remembered while she was going over the incident with the doctor. She said it was not a puddle of oil she slipped on it was actually a pair of scissors someone had dropped on the floor. Is there a way for us to amend the incident report to reflect this or will we have to re submit a new one?
