Employee States: TM was traveling on his jack to the next slot. He stepped off his jack to approach the slot and slipped on ice snow in the [REDACTED] breezeway hitting their head on the right side eye brow area. TM landed on the right side. TM does not believe he lost consciousness got up right away and contacted a TL. AP was contacted immediately. Pain is 8 10. TM also complained of right side hip discomfort.
