On [REDACTED] at approximately [REDACTED] Outfitter [REDACTED] working as a Pick Mod Packer was involved in an incident that occurred near the pick mod. Response at approximately [REDACTED] included first aid and a call to CorVel. The incident was reported to [REDACTED] then to [REDACTED] at approximately [REDACTED].
