What happened

The employee reported that while removing the toolbox from the company truck employee slide across maintenance shop. The employee pulled from the straps; and the strap failed and broke. As a result the employee was forced to abruptly react losing balance and falling backward onto their tailbone sustaining a direct impact and contusion.
