Retrieving fallen mop bucket

What happened

Employee states that while cleaning the mop bucket fell over off of cart. Employee leaned to pick up the bucket and tripped over the edge of the cart. On assessment post event bucket was bigger than space allowed for on cart. If bucket were fitted to space on cart future event could be prevented from occurring. - PMA intake EE states she was walking past the cart and tripped over the cart and fell and hip R side shoulder and butt.
