On the morning of [REDACTED] [REDACTED] was working in [REDACTED] receiving area unloading trailers operating an electric powered stand-up forklift. He had just finished his morning break where he was in the breakroom from [REDACTED]. At [REDACTED] [REDACTED] comes to a complete stop on the machine on the receiving floor in front of dock door 206 then at [REDACTED] he reaches up and grabs his head and falls onto the ground landing backwards and hitting the back of his head on the cement floor. At [REDACTED] it appears that he is having seizure like movements as he is laying on the ground.

At 8:31:08am [REDACTED] comes to a complete stop on the machine on the receiving floor in front of dock door 206 then at 8:31:31am he reaches up and grabs his head and falls onto the ground landing backwards and hitting the back of his head on the cement floor.
