What happened

The Brief On [REDACTED] at approximately [REDACTED] a Wayfair associate while driving east-west in aisle [REDACTED] collided with another elevated Powered Industrial Vehicle (Order Picker) that was actively picking. The associate immediately reported the incident involving both PIVs. The Associate’s Description of the Incident The associate (Driver 2 operating Order Picker #14 Serial #[REDACTED]) states that while working in aisle [REDACTED] at location [REDACTED]-[REDACTED]27 and while picking product from an elevated position they felt a sudden impact when another associate’s Order Picker (OP #55) collided into their equipment while they were attached to the wire guide. Pain Scale: 8.5 out of 10. The associate operating OP #55 stated that while traveling down the aisle to drop off picks they collided into another PIV which was elevated and actively picking.
