What happened

A pallet was stuck in between section 3-9 and 3-8 of lower conveying so employee grabbed a pallet puller and began trying to pull the pallet back to section 3-8 the pallet puller slipped out of the pallet which made him slip and fall hitting his lower back on electrical box on line 5 lower conveying.
