WORKING ON REMOVING THE CHIPPER GUARD

What happened

EMPLOYEE WAS ATTEMPTING TO REMOVE COVER ON CHIPPER AFTER OTHER EMPLOYEE NOTIFIED THEM THAT IT WAS CLEAR RO REMOVE. MACHINE WAS STILL ROTATING AND SHOULD NOT HAVE BEEN REMOVED. THIS RESULTED IN THE ROTATING MACHINE TO IMPACT THE COVER CAUSING IT TO THROW OPEN AND IMPACT THE EMPLOYEE KNOCKING THEM OFF AN ELEVATED PLATFORM ABOUT [REDACTED] HIGH. THE FALL RESULTED IN A CONTUSION TO THEIR SHOULDER AND RIPS ON THEIR LEFT SIDE.
