Before the incident

[REDACTED] was walking from her workstation (Dryer #4) to the bathroom. On her way she stopped to say hi to coworkers at the catch-end of Dryer 2.

While walking by a large broken piece on a pallet skid (not OSB or plywood) caught her arm directly.

Injury or illness

received large wood splinter on the bottom of her R forearm. Deep splinter 2-3 inches long completely embedded in the skin. Employee in lots of pain and nervous. Refused First Aid
