Before the incident

Employee was operating an assembly machine stopped the machine to clear out a jam of parts wedged underneath the pusher and finger holders.

What happened

Instead of putting the machine in manual mode the operator left the machine in automatic mode. She was clearing the jammed parts and kept one hand in the moving parts then pushed the start button to try and reposition the machine for better access. The machine began to run at full speed since it was still in Automatic mode pinching the operator 's hand between the pusher slide and the table.
