Before the incident

Extending the hospital 's dock plate into his trailer.

What happened

Driver was walking into the trailer on the clinic 's dock plate that was resting on the trailer liftgate and inside of the trailer when the lip of the dock plate slipped out of our trailer causing him to fall between the dock and trailer. He fell on the right side of his upper body injuring his right elbow arm and shoulder.
