Before the incident

On [REDACTED] around [REDACTED] a [REDACTED] EE was picking 23 pound boxes out of flow rack location [REDACTED] As EE was bent over picking up a box a stack of boxes towards the back of the pallet was destabilized and fell onto [REDACTED] neck and the left side of their face. Another EE witnessed this and immediately retrieved a supervisor. EE was evaluated by ERT and the OHN and sent via ambulance to the ER.
