What happened

attempting to draw blood sample saw blood return and blood flowing to tube. Tub malfunctioned and began leaking removed needle and slide safety guard to cover needle. Cleaned up leaked blood and grabbed needle without knowing the needle cover malfunctioned resulting in a needle stick to right thumb.
