Before the incident

Employee stated that while she was walking into the garage on the first floor and she stepped onto a pothole and tripped in a pothole which caused her to stumble a couple of feet and fell fully onto her left side of her body hitting her left shoulder on down to her knee and ankle. I was in full sprawled out position. She scraped her left knee bruised the left shoulder elbow muscle spasms in right buttock and back. Pain bruising and swelling to the left knee calf shoulder elbow ankles right buttocks and back.
