Before the incident
Associate was working and was walking between the bathroom and pt tray. As the associate was walking through the bathroom door flung open and caused mild superficial skin abrasions on left side of abd approx 2.5 inch in length and 1 4 in in width. reports mild LUQ tenderness when twisting or reaching. Overall feels better since yesterday but still bothered with reaching twisting motion when performing job tasks. At the initial injury on [REDACTED] 25 associate reports due to sharp pain felt like 'passing out form pain '. on [REDACTED] 25 pain now is 5 10 with motion and guarded by pain. on no meds.
