EMPLOYEE WAS LOADING A SPLIT RACK APC. BEFORE LOADING THE APC HE DID NOT USE THE SECURE LATCH TO LOCK THE TOP SHELF OF THE APC. WHEN HE LOADED THE APC THE SHELF FELL AND THE BAR ON THE SHELF HIT HIM IN THE BACK OF THE HEAD.
