At [REDACTED] employee arrived at [REDACTED] to complete his delivery at [REDACTED] As he parked and gathered the packages he exited the truck safely then grabbed the packages for delivery As soon as he stepped on the curb he slipped on a sheet of clear black ice went about 5 inches off the ground and landed on my back side (left hip arm and knee) landed on the concrete very hard All the packages went in the air as he tried to break his fall He immediately started experiencing pain in his back left knee and shoulder and arm He called dispatch and LMET There were construction workers present at the address [REDACTED] who witnessed it and 2 bystanders who helped him off the ground
