What happened

While driving down [REDACTED] a lady did a U turn from a parking spot on the right side in front of the truck. This caused the driver to slam on the brakes to avoid a collision. This resulted in the employee to be forced forward into the dashboard windshield and the floor. The seat belt in the truck does not work. The woman who caused this fled the scene.
