WHIILE GRADING CONCRETE AS [REDACTED] WAS BEING POURED EMPLOYEE ATTEMPTED TO STEP OUT OF AY OF THE CHUTE. HE MISSTEPPED AND LANDED IN THE UNPOURED STREET BASE. THS CAUSED THE EMPLOYEE TO FALL BACKWARDS LANDING ON HIS LEFT SHOULDER AND THE LEFT SIDE OF HIS BACK. EMPLOYEE TRIED TO SHAKE IT OFF ALERTED GB WENT HOME RESTED BATHED. BUT WOKE UP [REDACTED] AM BARELY ABLE TO MOVE.
