What happened

[REDACTED] was positioned at the bottom of the loader transitioning to a transporter. To gain leverage [REDACTED] positioned his right foot on the corner of the loader. [REDACTED] (loader operator) raised the platform in an attempt to help the transition then lowered it. As the platform was lowered [REDACTED] 's foot repositioned and [REDACTED] 's Right big toe was crushed by the corner of the platform. [REDACTED] was wearing Steel toe boots as well as other required PPE.
