What happened

The employee was preparing to put wheels back on the fruit feeder machine. The wheels were off and up on blocks. A dolly moved and shifted the right block so when he laid on the dolly to go under the machine the dolly stuck the block on the right side again which caused the machine to fall on top of his upper body.
