Before the incident

Employee was walking up a set of steps to get up to the stacker cell where the glass is stored.

What happened

A safety gate that prohibits access to the cell if the fall protection gate isn 't closed was not moved prior to attempting to access the area. Employee walked into that gate knocking him off balance and he fell back down the small set of stairs hitting his head on the floor.
