Before the incident

Patient has a diagnosis of dementia at around [REDACTED] patient started to become more confuse and restless. patient was trying to get out of bed and try to leave the unit patient started to scream Bite Punch kick and scratch everyone around.

What happened

While the patient was kicking for patient safety i was holding the patient to prevent her from falling in that moment i felt a sharp pull in my back. I then apply Ice pack and inform my ANM of the incident.
