Before the incident

Injured worker was using a [REDACTED] pallet jack to load two pallets inside of a refrigerated trailer.

The refrigerated trailer had a bulkhead secured in an upright position and strapped to the sidewall. As the injured employee proceeded to drive into the trailer the bulkhead fell forward and struck the employee in the back causing injury.
