Before the incident

Associate was operating a lift truck and was on his way to see his supervisor.

What happened

Associates stated that their lift trucks impacted one another. What I observed was lift truck [REDACTED] backing from a trailer in [REDACTED] in [REDACTED] the pad lift was traveling south down the dock. As the pad lift truck was approaching door 361 lift truck [REDACTED] backed into the pad truck causing the lift trucks to collide with one another. The pad lift truck impacted lift truck [REDACTED] on the right rear causing some damage to the battery restraint. I have advised the driver of [REDACTED] to look back before proceeding from a trailer. The driver of the pad lift stated that he has a headache after the impact the other LTO stated that he was OK. Maintenance came and inspected the lift trucks and found damage on lift [REDACTED] and it was taken to the shop for repair the pad lift checked out OK.
