Assisting a forklift operator with picking and packing mooring anchors.

What happened

Both EE 's already placed four anchors on a pallet and when the fifth one was placed lying down in between [REDACTED] noticed the 'bell ' end was sitting on the edge of the pallet. [REDACTED] attempted to roll the anchor onto the pallet instead it slipped off the pallet causing the shaft of the anchor to make contact with [REDACTED] 's head.
