Associate states that on [REDACTED] at around [REDACTED] he was pushing loads from the paint booth into the oven and he began to feel a jolt like sensation of pain down his lower back to his hips and into the front of his legs mainly on his quadricep muscles on both legs. The associate reported the injury on [REDACTED] prior to his shift at around [REDACTED] due to the pain he still felt. Associate came back to work on [REDACTED] and got with his supervisor to call Med Cor. Med Cor recommended to be scene by a medical provider. Associate will Go to Concentra on [REDACTED]. for further evaluation on his potential injury at [REDACTED] [REDACTED]. ; ; Employee returned to work at approximately [REDACTED]. Concentra clinic placed the associate on restricted work for the following 4 weeks and prescribed muscle relaxers for the symptoms. Medical professional instructions for the employee are as followed May lift up to 20 lbs occasionally; May push pull up to 20 lbs occasionally avoid stoopin
